New Patients

Please download, fill out, and bring the New Patient forms to your first visit.  You will need Adobe Acrobat Reader.

About your first visit

You will be evaluated by one of our physical therapists.  From there we will come up with a game plan that will maximize our chances for success, whether it's manual therapy (soft tissue mobilization/massage, joint mobilization), exercises/stretches, patient education in posture, body mechanics or movement strategy, modalities (ultrasound, electrical stimulation), or a combination of all of the above.

We suggest that you dress comfortably for your first visit. Athletic wear (shorts, t-shirt or sports bra) is recommended.

Please bring the following:

• Physician's referral
• X-ray or MRI reports (if applicable)
• Insurance card

Your first visit will take about 45 minutes to 1 hour.
